Personal Care Aide (PCA)
A Personal Care Aide, under the supervision of a Registered Professional Nurse, is responsible for assisting patients with non-medical personal care and home management activities (within the limits of the patient’s condition.) The PCA must be able to provide covered services according to the client’s care plan. Job functions may include:
- Making and changing beds
- Light household cleaning
- Laundry, including necessary ironing and mending
- Payment of bills and other essential errands
- Bathing of the client in the bed, the tub, or the shower
- Dressing and grooming
- Toileting, which may include assisting the client on and off the bedpan, commode or toilet
- Assistance with durable medical equipment
- Walking and transferring from bed to chair or wheelchair
- Preparing meals
- Assisting with administration of medications
- Providing routine skin care

Full time position available in our Menands office to assist prospective participants to become waiver participants and to coordinate and monitor the provision of all services for the TBI Waiver and the Nursing Home Transition and Diversion Waiver programs. The Service Coordinator’s goal is to increase the participant’s independence, productivity and integration into the community while maintaining their health and welfare. A Bachelor’s degree in a health or human service field with two years’ practical experience providing service coordination for individuals with disabilities is required. Must be able to work independently, demonstrate organizational / time management skills, sound decision making, and have excellent written and verbal communication skills.
RN Care Manager – MLTC Program
Full time position available in our Menands office for an experienced R.N. leader to direct the delivery of care management services for the chronic, long-term care population and coordinate the services of the MLTC program. The R.N. Care Manager ensures regulatory compliance, adherence to protocols and standards, quality of services, and efficient care delivery processes. A B.S.N. in Nursing or a Registered Nurse with relevant experience. Experience in the administration and management of long-term care programs or certified home health care highly desirable. Experience as an R.N. Care Manager preferred.
Per Diem LPNs – All Locations
LPNs needed to assist in provision of home health services to patients. Various hours, days, and shifts available. Job functions include coordination with RN, physician, and case manager in planning of patient care, providing direct patient care as per care plan, ensuring home safety, and documentation of care provided. Current NYS licensure as an LPN and a minimum of one year experience in a hospital, nursing home, or home care setting required. Home care experience preferred.
