Visiting Nurses Home Care is approved for service coordination and as a provider for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waivered Programs.

The NHTD Program is created to offer New Yorkers with disabilities and senior citizens a way to live in the community instead of a nursing home. The TBI program provides services to persons with a traumatic brain injury and helps them live in a community setting of their choice.

The New York State Department of Health oversees these waivered programs and contracts with “Regional Resource Development Centers” (RRDC) to manage the waiver throughout the State. Each RRDC has a contact person, the “Regional Resource Development Specialist” (RRDS) who can explain the program and help people get started, as well as assist them in selecting a Service Coordination agency. Visiting Nurses Home Care is a Service Coordination agency.
