NHTD/TBI

Visiting Nurses Home Care is approved for service coordination and as a provider for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waiver Programs.

The NHTD Program was created to enable New Yorkers with disabilities and senior citizens to live in the community instead of a nursing home. The TBI program provides services to persons with a traumatic brain injury and helps them live in the community setting of their choice.

The New York State Department of Health oversees these programs and contracts with Regional Resource Development Centers (RRDC) to manage the program throughout the state. The RRDC staff can explain the program and help individuals get started, as well as assist them in selecting a service coordination agency. Visiting Nurses Home Care is a service coordination agency.
